Open Position: Associate Director of Medical Case Management - Full Time

Responsibilities: Conducts new patient interviews to assess, develop, and implement an individualized patient care plan. Makes referrals to supportive community agencies and coordinates services to ensure continuity of care. Acts as liaison between hospital social workers and ACHC staff for hospitalized patients. Assumes role of advocate for patient and family. Provides HIV testing as per the NYSDOH guidelines. Develops and monitors the case management staffing schedules and assignments. Works with director to monitor and ensure compliance with AIDS Institute Case Management standards. Assists in the planning and development of programs and services for case management. Ensure the integration of social work/case management services into the multidisciplinary ACHC clinical team structure in order to support a holistic, coordinated and patient centered HIV care delivery experience. Work with director to design systems for measuring productivity, establish workload targets and evaluate outcomes.

Education & Experience: Masters Degree in social work and NYS Licensure (LMSW) required. Fluency in Spanish is preferred. At least 3 years experience working in a clinical or medical setting. Must be certified by the NYSDOH in HIV counseling and testing within six months of hire. Previous administrative and supervisory experience needed. Must be accustomed to diversity.

Open Position: Systems Administrator

Responsibilities: Windows Server Update Service administration and patch testing. Terminal Services administration and troubleshooting. Software license administration. Website development and maintenance. Support, maintenance and troubleshooting of hosted VOIP system. Cell phone account and hardware management, upgrades and replacements. User training. VMware Server administration. Daily monitoring, maintenance and troubleshooting of SQL Server 2005, Exchange Server 2007, Windows 2003 Servers, Microsoft Office Sharepoint Server 2007, and network applications. WAN/LAN troubleshooting.

Education & Experience: At least 4 years of desktop and technical support experience. At least 1 year of systems management experience. Strong understanding of Windows network environment, group policy, and organizational units. Experience in a medical environment, or medical knowledge is a plus. Familiarity with crystal reporting a plus. Must be able to lift 50 lbs. Education level: Minimum 2 year degree. 4 year degree strongly preferred.

Open Position: Pharmacist-Full-Time

Responsibilities: Participates in the day to day operation of the Pharmacy activities. These activities include, but are not limited to prescription processing, customer service, appropriate ordering of pharmaceuticals and supplies, and accurate patient and 3rd Party billing. Performs direct supervision over all pharmacy support personnel, and fulfills the role of Pharmacy Director in his/her absence. Performs direct clinical pharmacy functions including dispensing, patient counseling, and clinical drug monitoring.

Education & Experience: Pharmacist D., current New York State Pharmacist License, experience with HIV/AIDS preferred.

Volunteer Opportunities

Thank you for your interest in becoming a volunteer! AIDS Community Health Center relies heavily on our dedicated volunteers to help us with administrative support, program support and fund raising events. We hope that one of these options interests you:

1. Administrative Support
Volunteers support AIDS Community Heath Center by donating their daytime, weekday hours performing clerical duties such as data entry, filing and copying. Our office is open Monday, Wednesday, Thursday and Friday 9 AM - 5 PM and Tuesday 9 AM - 7 PM. If you are interested in volunteering in our office, please contact Adrienne Jophlin at ajophlin@achcRochester.org or at 585-244-9000.

2. Program Support
Program volunteers assist Community Services with outreach, which could consist of representing AIDS Community Health Center at a Health Fair or outreach event, or assisting the outreach team during street outreach. If you are interested in helping with our outreach effort, please contact Lucia Colindres at lcolindres@achcRochester.org or by phone at 585-244-9000.

3. Fund-Raising Events
AIDS Community Health Center conducts several fund-raising events every year.
